Registration of past births

Required Documents

i. Declaration under section 24 (LT 6)

ii. The result of registering that births were not registered

iii. One of the following documents, including the date of birth and place of birth of the person

• Birth certificate of the hospital or a certificate of the hospital

• A report of the Grama Niladari or a certificate from the Grama Niladhari

• Estate birth record or certificate of the estate

• A copy of the birth certificate of the Family Health Officer

iv. If there are no above documents, one of the following documents

• Health development note

• Baptism certificate

• A certified copy of the school enrollment register for the first time

• the record Student's work

• A valid identity card

v. If the parents are married, their marriage certificate (if the father accepts the fatherhood when the parents are unmarried, the father's affidavit and the signature of the father and father of the person in the sixth statement of the 10th statement)

ii. The results of the search for deaths have not been registered

iii. One of the following documents, including the date of the death of a person

• Hospital report or a certified copy

iv. Obituary Notice or condolence

v. The letter issued by the clergy that participated in religious activities

vi. Bills of the organizations that provided funeral services for funeral services

vii. Letters issued by the welfare society to the effect that the death of the deceased has been paid out gratuity

viii. Documents to prove the death of the deceased (birth certificate, marriage certificate, birth certificate of a child)

ix. Documents that can be proved are the dead persons who are engaged in any job or profession

x. Medical reports confirming the cause of death
